Memory Jogger Questions to Assist in
                Writing Personal Histories

It is not intended that one should answer all of the questions in this booklet.  It is done in outline form.  If a particular subject is not applicable or if you have no information on that area, skip to the next area for which you have information. 
 
Let your history reflect who you really are.  Don’t bog down in answering all questions or worrying about proper English.  Don’t gloss over your accomplishments or delete the hard times in your life.  Both are part of life.
 
My personal physician emphasized that the medical history, while not very interesting, could be important in future medical treatment.  He said it would help him a lot if his patients knew more of their family medical history.
